Welcome Homeschool/ESA Families! 

If your student would like to participate in Cooley Middle School athletics, please complete the following steps in order:

1.  Verify Residence - Contact Cooley Middle School at 480-279-8300 to verify that your home address is in the Cooley Middle School boundries.

2.  Complete Enrollment - Enroll your student through HUSD.org.  Your student will be enrolled as Athletics Only.  During the enrollment process, you will be asked to upload the following documents for your student:  birth certificate, immunizations and proof of residency.

3.  Complete Aktivate Registration -  complete the online athletic registration process at aktivate.com.  Detailed instructions can be found under the Athletics tab on the Cooley Middle School website.

**Important:  Steps 1-3 must be completed before the first day of tryouts.  Students who have not completed all required steps prior to tryouts will not be eligible to participate in tryouts or the athletic season.

4.  Approval Confirmation:  Once Steps 1-3 have been completed, the registrar or Athletic Director will contact you to confirm your student's eligibility and approval to participate in tryouts.
